Two Dogs & A Goat Incorporated (the "Company") agrees to provide Dog Walking & Exercising services for you and your pet(s). The Dog Walking & Exercising services will be performed __________________________________. The total number of visits will be ____________________ each week.

You will be charged $__________ per walk unless otherwise specified. Payment for the Dog Walking & Exercising services is due upon receipt of your invoice. So as to not be charged, all cancellation must be made twenty-four (24) hours in advance and must be acknowledged by the Company.

You will be charged $__________ as a deposit fee; these monies represent two weeks of services rendered in accordance with our bi-monthly billing cycle. All deposited funds will be applied to your final invoice.

Services will be performed in accordance with the instructions contained herein. The client waives any claims against Two Dogs & A Goat Incorporated unless the Company is negligent and does not perform as agreed herein.
